Do you have a Subpoena form anywhere on the site for Ohio?
Specifically, I am looking at requesting documents from Time-Warner in Akron.

I found a subpoena to a third-party for documents and filed it certified via the clerk in my county.
I was seeking my records and the previous tenants records only to show that cable infrastructure such as installation, coaxial and lines were in the apartment before I became a tenant.
Service was first Adelphia and taken over by Time-Warner. The subpoena was sent to Time-Warner of NE Ohio admin. offices.
They have ten days to respond one way or the other. However...........
If I don't hear anything or receive a reason why they won't supply the information do I have a recourse and do you have a form or document to file in that case?
The lease said I was allowed cable now the former landlord wants to replace paneling throughout because of a drilled hole where the cable comes through.
